Direct Primary Care is a membership-based healthcare model where you pay a flat monthly fee directly to your doctor: no insurance middleman. That fee covers unlimited primary care visits, extended appointment times, and a real ongoing relationship with your provider.

Our Direct Primary Care memberships do not require insurance. You pay a simple monthly fee and that covers your primary care. Insurance can still be used for labs, imaging, specialist referrals, and hospital care when needed.

Memberships at Midwest Primary Care start at $100 per month for one adult (ages 14 and older). Two adults pay $175 per month, and a family of up to four is $250 per month. You can also pay annually and save the equivalent of two months.

Your membership includes unlimited visits with your provider, fast scheduling, extended appointment times, preventive care and annual wellness exams, chronic disease management, coordination of labs and specialist referrals, access to your provider during working hours, and discounts on labs, procedures, and additional services. Direct Primary Care members also receive free urgent care visits.

No insurance is required to join. Membership is open to anyone 14 and older regardless of insurance status. Your monthly fee covers all primary care visits directly.
